As a Team member for ND Department of Health & Human Services, you will ensure the most vulnerable have access to a high-quality healthcare system in which prevention works, treatment is effective, and people recover.As a Case Manager for the Med Only and Crisis Team, you will provide individuals opportunities to manage their own recovery so they can live and work independently in their community.With the support of NCHSC Nurses, Medication Providers, and Crisis Team members, you will conduct comprehensive needs assessments, complete and implement service plans, as well as leverage contacts and complete referrals to community partners to ensure access to needed services for individuals on your caseload.Case Management services may be provided in the individual's home, community, and/or the office.
